New Clients/Intake Packet & Insurance Super Bill
Welcome. Thank you for taking the time to read and fill out the following. The intake packet and procedures are very typical for entering psychotherapy. I look forward to meeting you. Important note: I only provide hardcopy Super Bills every four sessions for people with PPOs insurances so you can be reimbursed directly, otherwise it is cash pay.
I have a waiting list, but I am very good at getting people in quickly. By texting/emailing me this intake packet it assures you a spot in the waiting list so that you can enter my schedule rotation as soon as possible. Anybody in a crisis is seen right away and the schedule is worked out later.
